The phrase "a bad item" is correct and usable in written English.
It can be used to describe an object, product, or concept that is of poor quality or undesirable.
Example: "After reviewing the feedback, we realized that the toy was a bad item and needed to be recalled."
Alternatives: "a poor product" or "a subpar item."
