In the UK, oesophagectomy is the current recommendation for patients with persistent high-grade dysplasia in Barrett's oesophagus.
Hemophagocytic syndrome (HPS) is a serious disorder characterized by persistent high-grade fever, hepatosplenomegaly, pancytopenia, abnormal coagulopathy, and hemophagocytosis by macrophages in the bone marrow, spleen, liver, and lymph node [ 1– 3].
Treatment is discontinued in the event of unacceptable toxicity, excessive treatment delay, disease progression, recurrent high-grade T1 tumours, or persistent high-grade or T1 tumours despite induction and four maintenance instillations.
The patient was treated with penicillin but had persistent high-grade fever until day 9 of admission, when he was no longer febrile and was then discharged in good condition.
Improvement in these parameters is crucial as the prevalence of cardiovascular disease and cachexia is higher in RA than in the normal population [ 10], partly due to the presence of traditional risk factors [ 25- 28] but also due to the metabolic and vascular effects of persistent high-grade inflammation [ 29, 30].
These findings may be attributed to the fact that the levels of blood glucose and HbA1c were unaffected by vitamin D supplementation, suggesting that the possible protective effects of vitamin D against diabetes-induced increase in aortic stiffness and endothelial dysfunction may have been abolished by the persistent high-grade hyperglycemia.
HPV types 16 & 18 in particular contribute to 74-93% of these cases [ 1- 6] and clinical studies of the quadrivalent HPV vaccine has proven protection against infection, persistent infection and low and high grade lesions in male and female populations, respectively [ 7- 9].
In the case of persistent or high-grade irAEs, ipilimumab should be permanently discontinued and patients should be treated with high-dose systemic corticosteroids (with slow taper to reduce the risk of symptom relapse).
On the other hand, if this bias exists when considering the whole group of patients, the same confounding factors cannot be advocated when dealing with the most pathologic cases, that is, those with persistent nocturnal high-grade abnormal UAE and those with macroalbuminuria, in whom a high percentage of remissions is confirmed.
Persistent high-risk human papillomavirus (HPV) infection is strongly associated with the development of high-grade cervical intraepithelial neoplasia (CIN) or cancer.
Only a small fraction of persistent HR-HPV infections will progress to high grade cervical lesions and cervical carcinoma.
