Items collected will include item numbers/description, including hospital and community items, type of specialty of provider, postcode of participant, postcode of provider and fee paid, including payments on Medicare-extended care plans which allow access to physiotherapy treatment and other allied health services.
Following this process, the pilot ARQ was revised to create the ARQ-Revision 1 (ARQ-Rev1) which was comprised of six scales and 79 items in the individual domain, and two scales each in the family (20 items), peer (11 items), school (15 items) and community (15 items) domains.
It consists of 28 items covering the six areas of the CCM Organizationn of the healthcare delivery system (6 items), community linkages (3 items), self-management support (4 items), decision support (4 items), delivery system design (6 items), and clinical information systems (5 items).
The measure of perceived importance of substance use prevention consisted of 6 items about prevention for justice-involved youth under community supervision; item wording appears in Table 1.
The initial draft encompassed six categories and 96 items including: educational resources (20 items), teaching and training plans and outcomes (43 items), research and results (9 items), development of faculty and continuing education (8 items), academic exchanges and the roles and function of the hospital in the community (8 items), and administration (8 items).
The seven multi-item scales were first contact and continuity (6 items); comprehensiveness (medical care) (4 items); comprehensiveness (social care) (3 items); first contact (access) (2 items); coordination (2 items); family centeredness (5 items); and community orientation (4 items).
The IFSAC consists of five subscales and 36 items, that is, infant care (6 items), personal care (8 items), household activities (12 items), social and community activities (6 items), and occupational activities (4 items).
Burckhardt et al. [ 21, 39] suggested that the QOLS comprising three sub-dimensions: relationship and marital well-being (items 3, 4, 5, 6, and 14); health and functioning (items 1, 2, 11, 15, and 16); and personal, social, and community commitment (items 7, 8, 9, 10, 12, and 13) [ 21, 38].
