In the ALDS project, 'not applicable' item responses occur when patients have never had the opportunity to attempt to perform the activity described.
During screening, references were marked as 'exclude' taking a hierarchy approach and marking only the highest applicable item on the list.
For example, "stands quietly when in line", an item commonly used to assess ADHD among schoolchildren in high-income countries, was not an applicable item for ADHD in Nepal because it was not a common task for some children [ 18].
Higher levels of reliability (alpha ≥ 0.9) are required of measures that are being used to compare an individual's scores across time [ 19] and for such purposes the full scale score would be needed with no more than one applicable item missing (excluding N/A items).
