Twenty-two candidate patients were excluded before randomisation: 11 did not meet the inclusion criteria (4 were institutionalised, 4 were not home care patients and 3 were difficult to follow-up), 5 declined to participate and 2 whose corresponding health professionals did not wish to take part in the study, while the other 4 died.
