We have added the adjusted p values to the models using the entire dataset and 2 adjustments, first for the 9 tests performed for Table 3 in the initial submission (3 of which were excluded in subsequent revisions), and for the 21 tests when including tests for force levels below and above 200 N. Analyses were completed in SPLUS 6.2 (In Sightful, Seattle, WA).

Two general approaches to control experiment-wise Type I inflation are: 1) statistical/design considerations that reduce the number of p-values, including factorial designs, multivariate techniques, and repeated-measures analyses; and 2) adjustments to the α level, including newer approaches that are less conservative than, for example, Bonferroni corrections.
Step 2: Adjustments due to interspecies differences and study conditions.
For the second model (Model 2), adjustments were made according to Model 1 plus, geographic variations (ESRD network) and facility length of ownership.
In Model 2 adjustments were also made for: sex; urban/rural area of residence; marital status (married/cohabiting vs. never married vs. divorced/separated/widowed), and wealth or education as possible confounders.

We introduced 2 adjustment models for Bayesian NMA.
Model 2 – adjustment for age and marital status.
We compared models with 1) no adjustment for adult size, 2) adjustment for adult BMI, and 3) adjustment for adult BMI and height.
In model 2, adjustment for measured height and weight were also included.
All associations were slightly attenuated, but remained elevated, after additional adjustment for ETS (Table 2, adjustment 2).
